The Web Mastering Learner will demonstrate a knowledge of how to use Dreamweaver to create a web page which uses a form to collect data that can be sent via email.

Acquisition of this knowledge will be demonstrated by the Web Mastering learner creating a web page that:

1. contains a form which collects information from site visitors
2. emails data collected on the form to an email address 
3. uses browser safe colors for background color(s)
4. uses a titlle
5. uses graphics

Assignment

Follow the directions included on the Creating an Email Form in Dreamweaver 4 handout to  prepare a web page.

• You may choose the subject and title of your survey
• Your form must contain at least 10 questions.
• Be certain to use all of the form objects (text field, check boxes, radio buttons, etc.) called for in the handout
• Your subject and questions must be 100% school appropriate.
• Use patr@ccisd.com as the email address to which your completed forms will be sent.
• You will begin on page 1 of the handoout, under Procedures, instruction #7
