 This section once completed will go in depth upon the Nurses role in regards to safety and quality. Nurses play an important role ensuring patient's are safe while in the hospital and also the community setting. Promoting a safe environment begins through the process of assessment

Expanded reflection of quality and safety

The definition created in the professional roles and values course is valid but need substantial expansion after successful completition of Courses, reading and performance of assessments while at Western Governors University. Although the previous definition correctly highlights the nurses role in creation of a safe environment will hospitalized and in community, it will take more than simply assessment to create a safe environment. Understanding the complexity of health care system in the United States will help to promote and improve delivery and quality of care. Continued costs and aging population will require significant changes in current model of health care. Understanding the importance of nursing sensitive indicators will also play an important role for the development and implementations to prevent falls, pressure ulcers and other hospital acquired ailments that may lead to negative health outcome and increased health outcomes. The ability to identify health hazards and complications and risks will help to improve delivery model of care. Nurses will need to be on the forefront of quality improvement projects to help reduce costs and improve patient care and patient outcomes.

A. Development of definition

The development of the definition can be attributed to the course work that was completed in C489 Organizational Systems and quality leadership. The completion of the Institute of Healthcare Improvement (IHI) and the completition of the three required papers outlined the importance of quality and safety and leadership. The ability to properly a root cause analysis will help to ensure the culture of blame is replaced with the culture of safety and continued improvement. The in depth analysis of health care in the United States played a significant role of the creation of definition. reduction of falls and pressure ulcers and nosocomial infections will be significant. The course of study highlighted these areas.

B. Artifacts

A previously mentioned artifact is the Institue of health care improvment (IHI) certificate which can be found in the appendix. The other artifacts and work included in the portfolio are the three required papers for course C489 Organizational Systems and quality leadership. The first paper goes indepth about the importance of nursing sensitive indicators. The second paper allows for a successful implemetation of a root cause analysis. The third paper compares the health care system in Japan compared to the Health Care System in the United States.

i. Artifact support for definition of quality and safety

The required artifacts and work significantly impacted the revised definition of quality and safety.  Understanding nursing specific indicators can have a positive impact on nursing care as there will be a greater emphasis on prevention of falls, pressure ulcers and utilization of restraints. A scenario is discussed in the SAT 1 and the patient develops a pressure ulcer because of excessive utilization of restraints. The task allows for identification of risk factors and leads to potential change in practice because of the importance of prevention of hospital acquired pressure ulcers.  The second paper outlines a root cause analysis and identifies system level gap. Policy and protocols should be revised and re-evaluated. The identification the failure mode and effect analysis was also important for the development of the definition because changes may need to be made because of events that may occur. The task also required the creation of a root cause analysis. The abiltiy to understand the complex nature of the United States healthcare system also enabled a more detailed definition due to the increased costs and need for changes to improve quality and reduce mortality

2. Institute of Healthcare Improvement (IHI) Certificate

The Institute of Health Improvement (IHI) certificate was attained through the successful completition of the IHI open school course that was required for course C489 Organizational System and Quality Leadership. This certificate was earned through the detailed modules and quizzes that instilled a higher level of analysis of patient care and quality improvement. The ability to utilize the tools included for a Root cause analysis will be important for identification of potential system errors. Continued improvement and quality care are two of the important tenants of the IHI course. The ability to understand the financial climate and environment will help to enable cost saving measures while also improving quality of care and patient satisfaction. identifying system level gaps will also help to improve patient care. The IHI certificate will have a positive impact on continued professional growth and health care improvement
