During your program at APU, you will assess your own dispositions as well as be assessed dispositionally by instructors and university mentors (if applicable).  The form below will be used by all parties including yourself.  The number of times this assessment occurs varies by program.  Instructions are given in the program DRF where these assessments are to occur.  

IMPORTANT:  The instructor's cue to complete his/her assessment is either by the completion and submission of your self-assessment or the completion and submission of the dispositions assessment request form in Taskstream.  Fulfillment of this requirement is crucial as you will undergo at least two transition point evaluations during your program when these assessments will be checked for completion and content.
