As prescribed by the program director for program you are teaching in, you may be assessing your students' dispositions in Taskstream.  The form below will be used by all parties in this assessment process including yourself.  IMPORTANT:  You should have been given dispositions assessment language for the course you are teaching, either in a master syllabus or by your program director or lead instructor.  Please contact one of these two individuals if you have not received dispositions language and information. 

The instructor's cue to complete an assessment is either by the completion and submission of a candidate self-assessment or the completion and submission of the dispositions assessment request form in Taskstream.  Fulfillment of this requirement is crucial as candidates will undergo at least two transition point evaluations during their program when these assessments will be checked for completion and content.
