Students: Expect to recieve an email from Providence with an Orientation Letter and advising to complete the Student Forms Packet. Submit the packet directly to Providence as indicated in the packet and provide a copy of the email clearance statement to your faculty or coordinator. 

1. HealthStream (Epic)
   • Faculty or Coordinator will provide you with HealthStream credentials and instructions to complete the lessons and associated competency examination; submit certificates of completion to obtain your network login username and password. 
2. Glucose Meter: AccuChek Inform II
   • Students: review the policy and education module, then complete the quiz with 90% of questions answered correctly and have a credentialed competency validator 'sign-off" on the competency validation form. 
   • Faculty or Coordinator: review the guidelines, policy, and complete/submit the access form. Please contact your designated SON Coordinator to obtain the quiz answer key. 
3. Review
   • Student Badge Process
   • PPMC Parking: cars parking in area other than those indicated for students will be ticketed
   • PSVMC Parking: parking is available in the North Parking Structure

Faculty or Coordinators:

• Providence - Oregon Region will send instructions to complete PRECEPTED clinical requests via email. 
• COHORT clinical requests will be facilitated through the process as advised by SMC with ACE-MAPP Oregon. Contact the Providence clinical placement coordinators or Alex Asbury, Regional Coordinator at SMC, if you have questions.
