Instructions: After clicking on the appropriate form link, you will have VIEW access in Google docs. You can then either download the file in Ms-Word format (clicking on "File") to modify it, or copy and paste the form into MS-Word.

You can also "Make a Copy" in the "File" settings if you would like to edit the document with a group of people (sharing the new Google Doc with them), but it still needs to be exported to MS-Word and sent to CPAC via email.

If you have any questions, please contact Patrick Blaine. Completed forms should be submitted to the CPAC chair.

Please note: This is a Google Docnot a Google Form. We are unable to use Google Forms because they output to a spreadsheet, which is not workable for the committee.
