MTTS form (form is completed online; documents are attached)

Portfolio

Publish portfolio in TaskStream
Submit portfolio URL in TaskStream
Email (FSU Email) URL to program coordinator at least one week before Portfolio Evaluation Night
"Pack-it-up" (directions in resources section), expand the file, burn on CD, submit to program coordinator (before portfolio evaluation)

Supervisor TaskStream Submissions

Score the MTTS V using the rubric provided
Score the unit plan using the rubric provided
Score the lesson plan using the rubric provided

Provide feedback for the portfolio throughout the semester
