Start with the outline on the last page of the attached Program Review Manual. 

Then take a look at the attached samples.  You can see that  the narrative sections of the manual have varied in length and style.  Depending on the story you have to tell about your programs and their accomplishments you can model your report after whatever makes the most sense to you.  Your report is for your programs and is not expected to conform to any particular format.
