The school has been using survey monkey for students to complete anonymous surveys for the past several years.  Each teacher reviews the results of the survey with the Director of the center and discusses ways to improve instruction.  Additionally, all completers are contacted one year after graduation and asked to answer survey questions.  The answers to this survey are submitted to the department of education.  The following artifacts are attached:
