Community and population health

This page contains evidence of my growth as a professional in the area of (enter area here).
Learning about the community and population health of our local community has given me a new perspective on the availability of alternative Health Care practices. When examining all of the community resources I have found that we have a diverse population, low-income children and seniors definitely profit from the resources. Not to say that a majority of the population has access to community health promotion. In the broad scope of nursing the clinical approach is increasingly costly to insurance companies and hospitals. Community-based resources help people to gain a continuum of care post hospitalization. Understanding diversity in a community health to promote individualized community resource availabity. I have gained a very good understanding through WGU of the importance of getting quality care throughout the diverse population. In palm beach county we have a very large availability of community resources, through community field work I have become very familiar with them. I was able to gain a real understanding for our community issues and how to help assess our resources for positive outcomes. My community field work was on obesity prevention, there are vast services that I've learned about to encourage understanding and goal attainment. Working in the cardiac cath lab we discharge our patients with instruction that include attending cardiac rehab, there are many programs in our community to help them recover fully. WGU has helped give me the knowledge of the community resources and that I appreciate greatly to further my ability to guide my pateints care.
