Letter of Recommendation for Isabel M Kozak, DNP, NP-C, CLNC

Date: 4/9/15

Name/Credentials: Gwendolyn Astudillo-Tee, MSN, FNP

How long have you known Isabel Kozak? 6 years

Reason(s) for recommendation:

When I first met Isabel Kozak at a nurse practitioner meeting, I was initially impressed by her enthusiasm, communication skills, and professionalism.  I decided to join her in orthopedics where she was the lead nurse practitioner a few months after that.

During this six years, she consistently demonstrated all of the above qualities and more.  She is reliable, committed to quality improvement, always upbeat. Her ability to manage her time, taking care of patients and managing the department is exceptional.  Isabel multitasks effectively and is able to handle high volume workload.

Isabel is organized, goal oriented and diligent making her a good multitasker. She can do her job as the lead orthopedic NP, develop new programs, and manage school with excellent grades. She completed her DNP and got certified in legal nurse consulting this year.

Isabel is hardworking, top performer, and will excel in anything she does in her field. She has my highest recommendation. Please don’t hesitate to call me if you have any questions.

Sincerely,

Gwendolyn L. Astudillo-Tee, FNP-C, Care Coordinator, Telehealth
