Student Permission and Privacy

The final three tasks require information about actual K-12 students. The Assessing Learning (3rd task) and Culminating Teaching Experience (4th task) tasks require you to submit samples of student assignments/work. The final task also requires an unedited twenty minute video of you teaching a lesson to students. 
 

You must have signed permission from any student (adults or a traditional K-12 students) before work samples of these individuals can be included with your submission and before recording the video in which any of these individuals may appear on camera.  Permission must be obtained ahead of time, prior to including the work in your TPA submission.
 

National University will supply you with sample permission slips to use for the students/adults in your classroom.  These permission slips are available to download within Taskstream.  Before using these forms, please double check with your school site to confirm these permission forms are appropriate to use in your K-12 setting. You should always comply with the policies and practices of the K-12 setting in which you are working.  Remember it will be your responsibility to protect the privacy of all individuals whose work you include with your task response.

Please note: You do not need to upload the signed permission slips. Please keep them for your own record.
