Once you have logged in, you will see this screen: 

Select the course (task) in which you would like to enter personalized course evaluation questions.  Select 'Show Section'

Once the section is open, the options available to faculty will appear.  The first three questions are single selection (multiple choice) questions on a five point Agreement Likert scale.  The second group of three questions are open-ended (free response) questions.  To activate the questions, you must click on "Select" on the right-hand side of the screen.  Then click 'Edit' to add the question text.

Once complete, click on Save (if you wish to save and return to complete your work later).  Or, click 'Submit' if you are done.  Note If you click 'Save' and do not return to complete your work, the questions will not be administered.

If you wish to use the same personalized questions in multiple sections, you can select 'Copy' or 'Copy and Submit' after selecting the section to which you wish to copy the questions from the drop down menu.
