You may be required to attach the following documents as PDF files using a scanner. Please note that because scanning and converting files to PDF format are procedures that occur outside of TaskStream, Mentoring Services can only provide limited support for these processes.
*Note: You will submit your work only when you have attached all the work for Planning & Instruction, Engaging Students & Supporting Learning, and Assessing Student Learning. Instructions for submitting your work are included below.
To upload your assessment for the Evaluation Criteria, click on the Assessing Student Learning link in the structure tree on the left side of the page. Click the Attachments button that appears at the bottom of the page in the Add Toolbar. In the Add New Attachment area on the left, click on the first radio button - 'A file saved on your computer'. If you are uploading more than one document, you must attach each document individually. Now you will click the Browse (PC users) / Choose File (Mac users) button to find the file you wish to attach. Once you have located your file, double click on the file name to close the pop-up window.
Name the file.
Add a description of the file, if applicable.
Click the Add File button when finished.
When your file upload is complete, it will appear to the right under Currently Attached Uploaded Files. If you need to add more files please repeat the process above. Once all files have been attached, click on the Save and Return button located at the top or bottom right corner of the page to be brought back to the Planning Instruction & Assessment page.
You will have the opportunity to view the attached file by click on the file name under Files in the Attachment Section. If you wish to add more files or delete a file, you may do so by clicking on the Manage Attachments buttons to the right of the Attachment Section.
If you have to make edits to a specific file that you have uploaded, you must make the changes in the original document and re-upload it to TaskStream by utilizing the Manage Attachments button located in the workspace for Assessing Student Learning.

To complete the Assessment Commentary section, you will need to complete the Assessment Commentary form. To access the form, click on the Assessing Student Learning link from the left structure. You can access the form in three locations. You may click on the Form button located at the bottom of the page in the Add toolbar, you may click on the gray Complete Form button located to the right of the workspace, or you may click the Complete this Form link located within the Form section of the workspace.
You may answer the form prompts by typing directly into the text areas or you may copy and paste your responses from another document. Please keep in mind that there is a limit to how many characters are allowed in each text box. The maximum allowed characters for each text box can be found to the bottom left of each text box. You can also verify how many characters you have use by click on the Count button located to the bottom right of each text box.
Please keep in mind that an asterisk symbol shaded in red, located at the beginning of each prompt, is a required field that must contain an answer before the form can be submitted.
When you have completed all of your form responses, or if you would like to save your current entries and return to the form at a later time, click on the Save and Return button located at the top or bottom right hand corner of the page. This will bring you back to the Assessing Student Learning work area.
You can view and/or edit your responses by clicking on the View/Edit Your Responses link located under the Form section in the workspace.
