When I began this program, I had begun another portfolio which was lost in the e-portfolio system crash. That information was lost, therefore I am incorporating material from another class that demonstrates problem based learning and technology. Since I am licensed in the state of Ohio, I have designed my lesson plans and assessments to Ohio academic standards. The lesson plan in the atached curriculum map incorporates the teaching of geometry principles and their application in several real life situations. This is an original curriculum map/lesson plan.  
AZ- Arizona's Professional Teacher Standards
Standard 3: The teacher implements and manages instruction that develops students' abilities to meet Arizona's academic standardsThe performance assessment shall measure the extent to which the teacher:
Assessed performance 1: Appropriately implements a teacher-designed lesson plan
Assessed performance 10: Connects lesson content to real life situations when appropriate
Standard 7: The teacher has general academic knowledge as demonstrated by the attainment of a bachelor's degree. The teacher also has specific academic knowledge in his or her subject area or areas sufficient to develop student knowledge and performance to meet Arizona's academic standardsThe subject knowledge assessment shall measure the extent to which the teacher has knowledge of:
Assessed performance 5: Connections between knowledge of the subject area and real life situations at the level of the students being taught

As explained previously, my work for this class was lost. At the time I took the class, there was no module 5. Therefore I am substituting this lesson since it meets the Arizona standard requirement. Part two of the curriculum map details formal and informal assessments that are aligned with the instruction. These include several performance tasks, quizzes, tests, work samples, homework and student self assessments.
