IMPORTANT

REQUIRED CLEARANCES FOR ENROLLMENT:

• Certificate of Clearance -- See attachment below for instructions.  You may also pick up a LiveScan application with Xochitl Castillo, West Faculty Bldg. #169.

• Liability Insurance -- Membership runs from Sept 1 - Aug 31 and may be purchased starting in Aug and any time in between.  Students returning in the fall semesters must renew their membership.  See email link below.

• Proof of Negative TB Test  --- tests must remain valid throughout the credential program.

• Release of Liability – Signature of this form indicating the Warning, Waiver and the Release of Liability of the University is required as part of the clearances for the DOE courses that may require participating in off-campus events. The form must  be picked up at the DOE offices and signed in front of a witness.
