Service Request Form

To request the following services, you and/or your students will need to complete an online service request form (available in the web links section below).

  • Rubric revision/ replacement/ deletion
  • DRF setup/ reorganization/ modification
  • Refresher training on data entry
  • Refresher training on assignment submission/ upload
  • Adding/ deleting resources to DRF's, Mybrary, Cybrary, etc.
  • Adding/ deleting adjuncts or field supervisors (i.e., Site mentors, University Supervisors, etc.)
  • Generating Program Activity Reports (descriptive stats on who has entered data and who has not)
  • Generating Performance by DRF Reports (Shows outcomes for individual or group on each item/requirement and/or assessment)
  • Small/ large program specific training sessions
  • Non-NCATE program set up in Taskstream (Going Live)
  • E-portfolio/ digital repository assistance
  • Request for training materials (i.e., instructional slideshows, recorded webinars, etc.)
  • Enrollment in additional DRFs (Directed Response Folio)
