Please make sure to adhere to the directions and type all documents. Also, you will need to have a professional passport picture, philosophy of teaching statement (please have your advisor check for content and accuracy), unofficial transcript, and official FTCE scores (unofficial may be submitted, but official score report will still be needed).

WHAT TO TURN IN:

YOU MUST SUBMIT TYPED, CLEAN, AND SPOT FREE HARD COPIES OF ALL MATERIALS!!!  FAXED APPLICATION PACKETS WILL NOT BE ACCEPTED.

            1.   One (1) copy of the completed Application Form

            2.   One (1) copy of the Personnel Form

            3.   One (1) copy of the Agreement Form for Transfer of Transcript

            4.   One (1) copy of the Course Information Form

            5.   One (1) copy of your transcript with grades including your last semester of attendance (Unofficial transcripts are acceptable)

            6.   One (1) passport photo in color **         

   7.    One (1) copy of the score report for the Florida Teacher Certification Exam (FTCE) (only if any changes since pre-application submission)

   8.    One (1) Statement of Teaching Philosophy

   9.   One (1) Signed Program Clearance Form (Elementary/Primary Education majors only)

  10.  One (1) Complete Major Field of Study GPA Form (Elementary/Primary Education majors only - submitted with pre-application)

**Photographs must look professional.  CVS, Walgreens, and Walmart have passport photographs that are acceptable and costs range from ($5.99 – $12.00) for one passport photographs. Men must wear a shirt, tie and/or suit jacket.  Women must wear a suit jacket and blouse.  Jewelry should be minimal.  No earrings for men.  Only professional looking photographs will be accepted.
