When a law (rule) is broken in my classroom students must fill out a Behavior Assessment.  The assessment is compiled of four questions,

1. What Law did you break?

2. Why did you choose to break the law?

3. What decisions will you change in the future?

4. Comments to Parents or Ms. McDaniel

I feel that these questions will give a sense of responsibility to the bad choices that my student made.  Once the student has completed the form, they must sign it, I must sign and their parents must sign it.  Once the form is completed and all parties have signed it, the student must turn it in to me.  I will be giving my student 2 days from the day I gave it out to return the completed form.
